best is by fax, 760.888.2032. 1. Download the document, save it, and
then begin to fill out the blanks. As you fill out the information, use your
underline feature... delete the extra lines you create and this brings the
document back to its original form with your information added. 2. Then send new email to: apassionforpaws@gmail.com and
attach your newly created document. 3. After we receive your foster agreement,
